Failure Mode and Effects Analysis (FMEA) is a systematic method for analysing risks and preventing faults in products, processes or systems. It is used to recognise and evaluate potential sources of error and weak points at an early stage and to avoid or reduce them through suitable measures.

Main objectives of the FMEA:

  • Systematic identification of possible errors and their causes: Early detection of errors and weak points in the system.
  • Assessment of risks: Examination of the significance, probability of occurrence and probability of detection of errors.
  • Derivation of preventive measures to avoid errors: development of measures to minimise risk.
  • Improving quality and reliability: Increasing the reliability and quality of products and processes.

Implementation of the FMEA:

The FMEA is typically carried out in interdisciplinary teams and comprises the following steps:

  1. Description of the system or process to be analysed: Detailed description of the entire system or specific processes.
  2. Identification of possible errors, their effects and causes: Listing of possible errors, their effects and causes.
  3. Assessment of the risks: Use of indicators such as significance, occurrence and detection to assess the risk.
  4. Development of risk mitigation measures: Development and implementation of risk mitigation measures.
  5. Implementation and review of the measures: Tracking and reviewing the effectiveness of the measures introduced.

Types of FMEA:

  • System FMEA: Analysing the interaction of different system areas.
  • Design FMEA: Focus on errors in product design and development.
  • Process FMEA: Investigation of weak points in production or service processes.

FMEA is a widespread method in quality management and is used in many industries, e.g. in the automotive industry, aerospace and medical technology. It helps companies to proactively manage risks, reduce costs and increase customer satisfaction.
